A referral to Adult Social Care is a request for an assessment on behalf of a specific person who has care and support needs who is either not known to the department or whose case has been closed, which is accepted by a member of staff as requiring further action. It can be made by anyone, either for themselves or on behalf of another person and can be made by telephone, letter, in person, or by email.
